Existing PDFunderlay not added in the entity

I have a CAD file with an inserted PDF. I used AcadSharp to read and write the same file, but the output file does not display the PDF, and the references show it as unreferenced. While debugging the entities section, I found that there were no PDFUNDERLAY entities. Could you check why it is not being added and fix the issue?

The Entities collection in the document's root only returns the entities in the model_space, is the PdfUnderlay in the model or in a paper layout?

In my files I can read and see all the PdfUnderlay in the documents.

For the output, I can see the pdf underlays in Autocad, the Xref status is unreferenced, that is something that I'm still investigating, but for what the documentation says, that flag only means that it cannot find any entity with that image, even when the image is displayed.

Yes, it's in the model space, but while reading this file, I couldn't find any entities in the file.

For the input the problem is that instead of using the regular dxf name PDFUNDERLAY the software that created the file uses PDFREFERENCE, I can fix that by adding the reference to the reader.
