Users can accidentally add the same item to the shortcuts panel multiple times, creating duplicate entries.
It would be much better for the prompt to change to "remove from shortcuts" or to be greyed out when the item is already a shortcut
How to reproduce
Add an item to shortcuts panel
Attempt to add the same item again
Observe duplicate entry is created
