fix(config): Update rate limit key prefix format

- Modify 'key_prefix' in exception-notify.php to use a more specific format.
- Update from 'exception_notify_' to 'exception-notify:rate-limit:' for clarity.
- Improve organization of exception notifications handling.
- Add phiki composer.json for improved development dependencies.
