feat: Svelte 5 support

I am currently using svelte-tel-input in my Svelte 5 projects but there is just some awkwardness around it since it is still technically using Svelte 4.

Would there be any reason this couldn't be moved to Svelte 5 (personal projects still using svelte 4 etc.). Just want to find out before I put in the time.

Let me know and I can start working on a PR!

The full rewrite has been already started a while ago.

Several IRL things have been holding me back lately.
Death of a family member, renovation of our first home, and so on.

I've opened a discussion topic about future changes: #235

I'm certainly open for any contribution. Just need to discuss about the way.
My "refactor" is based on previous mistakes and real world usage experiences.
