This makes the client more flexible, allowing us to reuse it for `index.crates.io` in the future.
This should make it easier to iterate on the implementation independently.
There is not much reason for us to pass in a custom `Client`. We can just create one and simplify the public API surface a bit.

I guess this is best reviewed commit-by-commit, since the PR performs a bunch of different small improvements to our Fastly API client code.
