pages begin at index 1, and end at NumPage()
See for example http://pdfkit.org/demo/out.pdf. Font dictionaries have the keys Encoding and ToUnicode. In this case, charmapEncoding should be used.
See page 146 of the PDF 1.7 spec: a page /Contents can be a stream, or an array of streams.
